IFBS FAMILY CASE MANAGER/VISITATION FACILITATOR– UPPER VALLEY/LEBANON, NH, VT AREAS (part time)
Child and Family Services seeks an Intensive Family Based Services (IFBS)/Visitation Facilitator. In this position, you will manage the case from point of referral to discharge as well as coordinating other services involved in the case. Working in partnership with the referral source and other providers, you will be responsible for maintaining communication with the referral source as well as the development and dissemination of all case related documentation. As Case Manager you would provide education for families regarding safe and healthy parenting skills. The Family Case Manager/Visitation Facilitator will be available for home and community based meetings as needed as well as facilitating supervised visitation meetings at our Lebanon, NH location. Bachelor's degree with 5 related years experience required, Master's Degree with 2 years preferred. The ideal candidate will have the ability to work independently collaborating with client and staff to coordinate services. Knowledge of Upper Valley area medical and behavioral services is helpful. Candidates need a flexible schedule including some evening hours (5pm to 6pm) in order to accommodate family needs. The Computer Help Desk Support Assistant will be responsible for providing the first point of contact for staff, as well as assisting in the management of the agency's hardware and software needs related to network and end users. A partnership between Child and Family Services and the Division for Children, Youth and Families (DCYF) was created to help with the completion of overdue child abuse and neglect assessments. Assessments will be completed out of the DCYF offices in Concord and will consist primarily of phone and computer work. ENHANCED CARE COORDINATOR – CONCORD, NH AREA (part-time)
Child and Family Services seeks a part time coordinator for the Family Support Program serving the Concord, NH area 20 hours per week. As the Coordinator, you will work in a team with a Family Support Specialist to provide care coordination for pregnant couples and families with children from birth to age three. You will work closely with Peri-natal and Medication Assisted Treatment providers as well as outpatient substance use disorder providers. Coordinators provide home and community-based support to build and coordinate individualized services for the child and families at risk. Extensive training and coaching is provided. Additional responsibilities include conducting specialized wrap assessments (evidenced based), identifying goals, social risk factors, family needs, visit coordination and preparation. Responsibilities include developing and implementing treatment plans as well as linking the family to resources in the community.
