Integrated Home Based Services (NH)

Overview

Integrated Home Based Services is a short term service that works to restore positive functioning in families who would benefit from a variety of coordinated services that are individualized to meet each family’s specific needs.    


Eligibility: Typically court referred

Areas served:  Laconia, Manchester, Concord, Dover, Derry, Nashua, Hooksett, Franklin, Goffstown, Plymouth, Rochester, Lebanon and surrounding areas

Services provided

IHB has discovered that the team approach works best as a direct response to the needs of families served by JPPOs, CPSWs and the courts.

Services provided include:

  • The family therapist helps the families to identify and achieve treatment goals, stabilizes families who are in crisis, builds on the family’s strengths and facilitates effective family interaction.  The therapist also connect families with relevant community resources that can further independence and success
  • The caseworker works with adolescent to assure that he/she is accountable to the courts’ order and monitors the youth’s behaviors at home, at school and in the community.  This can include random drug screening, curfew checks, and callbacks and education support (school attendance and performance tracking).  Caseworkers build rapport with youth through positive role modeling, encouragement and mentoring

The caseworker can help families learn practical parenting skills, effective discipline, care and nurturing, household organization, budgeting, nutrition, health and hygiene.

Crisis Response

Child and Family Services provides on-call coverage 24-hours, 7-days/week.  All clinicians are trained in crisis intervention and response. 


Reporting

The referral source is kept informed of progress through ongoing contact including monthly reports that address areas of work, progress and concerns as well as plans for future intervention. 


Fees:  $57.36/day, for the duration of service
